DEVELOPING AN INFORMED CONSENT FORM ACTIVITY

 

 

Checklist:

 

  1. Who is the investigator(s) and what is affiliation?

 

  1. What is the purpose of the research study and why is it important?

 

  1. What are experimental procedures? How long will it take? What will the participant be expected to do? Where will the experiment take place?

 

  1. What the potential risks/discomforts? How are these risks managed?

 

  1. How will the findings of this study benefit the participant and the public?

 

  1. State the terms of confidentiality/anonymity.

 

  1. Inform the participant of the right to withdraw at any time without penalty.

 

  1. State how the participant will be compensated for participation.

 

  1. Inform the participant of the investigatorŐs willingness to answer any questions. Provide a contact for the participant to request follow up information.

 

  1. Provide a place for the signature of the participant and date.
