National Championship Bid Forms

Please read the Letter from MUT to get more information about the bidding process. Completed bids should be mailed to Janice Anderson at the address below.

Print out the bid form and fill out manually or save it onto your computer and fill out electronically.  You can email it to Janice Anderson at jslug@mindspring.com.  Email copy will be accepted without signatures but a signature copy is required.  You must mail the signature page to Janice as soon as possible.
