Submit this form to request:

audio/visual setups for on-campus events
captioning (*Attn FACULTY: If you are requesting LIVE captioning for a Zoom online class, please fill out this pdf and attach to your Media Request Form: Live Caption Booking Form)
video recording of your event
poster printing, laminating, or graphic design services
photography
graphic design services

Please submit your service request as far in advance as possible to guarantee technician availability.

Please be as detailed as possible in the 'Description' section of the form.

If you are requesting Photography, you must have all student participants fill out a Talent Release Form.
