Certification Form (PDF Document)
Attention Fall, 2006, Transfers: UCLA wants your
certification forms by mid-February, 2006. You must turn them in
to the SHP Office in January and the process is time-consuming.
Start it right away.
Download the TAP form to
your computer (link at bottom of this page) , fill it out on screen, print it out and sign it and
return it to the SHP Office in Admissions and Records.
We will verify accuracy and
ask a counselor to review it. We will forward it to UCLA.
How does TAP work?
When you apply to UCLA as a SHP student you receive special
priority consideration for admission, which greatly increases your
chances of being accepted. To receive this benefit you must submit
the following form to the SHP Office in January if you plan to
enroll at UCLA the following Fall. The SHP director and counselor then certify
that your information is accurate and transmit the form to UCLA.
Pay careful attention to this form and this deadline; they may make the
difference between succeeding and failing in applying for admission to
UCLA.
Start getting ready now so you will not have to gather all your old academic records at the last
minute. Use this form as a guideline for what courses you will need to gain
acceptance to the major of your choice at UCLA. You should indicate an alternate major. If UCLA cannot admit
you to your first-choice major they may be able to admit you to your
second choice, but they cannot do so unless you have told them of your
second choice. List preparation courses for both your primary
major and your alternate major
Note also that you do not need to have completed all of your
required courses, or all of your six SHP courses at the time you submit
this
certification form in January. So long as the form shows that you are
enrolled in the courses you will need to complete
the requirements, we can certify to UCLA that you are "on
track" to completing the requirements.
IMPACTED MAJORS: Some
majors (click here for
list) are extremely competitive and your likelihood of being
admitted will depend strongly on how much of the major preparation you
have already completed and how strong your GPA is, not only cumulative
but in your major preparation.
In General, UCLA will not
allow you to take
required courses in the summer immediately before you transfer.
