Instructions for Graduates Relocating to Another State and require proof of Local Anesthesia requirements
Graduates of Cerritos College who are relocating to another state and need to provide proof that they have met the state’s local anesthesia education and training requirements must first obtain a copy of their official transcript through the Cerritos College Admissions & Records Office. The transcript will be used as documentation of completed coursework and training. Please contact the Admissions & Records Office to request the appropriate transcript and email a copy to Diane Loera for review and processing. Before requesting the transcript, carefully review the dental hygiene licensure requirements for the state in which you plan to practice and provide clear instructions regarding the type of documentation required. This should include whether the state requires an official transcript with the college seal or stamp on a hard copy, whether a secure digital transcript is acceptable, and whether the state requires completion of any specific forms, applications, or additional verification documents. It is the graduate’s responsibility to confirm the receiving state’s requirements and provide all necessary instructions so that the appropriate documentation can be prepared and submitted correctly.
