Volunteer Note-Taker Form
New note-taking information - We are in the midst of transitioning to a new type of notetaking service (Genio). Some SAS students are still approved for volunteer notetaker, so please double check the students accommodation letter prior to submitting this form.
If student is approved for "Use of notetaking app" (Genio), you do not need to complete this form.
If the student is not yet proficient in Genio, they are encouraged to schedule a training
appointment at the SAS office.
If student approved for "Volunteer Notetaker" you can proceed and submit the request form below.
Prior to completing this form, please ensure that this service is indicated on the
SAS students approved accommodation letter.
