International Student Forms

International students will find all forms and documents that are available in the Office of International Student Services (OISS). Many of these forms are available online, but if you required or prefer a paper form, please visit us in person for support.

Online Request Forms

The following forms are E-forms that can be submitted online to the Office of International Student Services. Please allow 3 business days for processing the request.

  • F-1 Student Information Update
    International students can update their contact information and preferred method of communication with the Office of International Student Services.

  • Enrollment Verification Request
    This form is used for students to request a letter verifying full time enrollment and maintaining F-1 status. It does not verify enrollment or serve as transcript of academic history.

  • Travel Signature Request
    This form should be completed by an F-1 Student requesting a travel signature on the I-20. The travel signature allows a student to re-enter the United States after traveling outside of the Unites States with a valid F-1 Student Visa. A Designated School Officials (DSO) at the Office of International Student will process your request.

This form should be completed by an F-1 student that is requesting to transfer their SEVIS Record/I-20 to        another college/university. A Designated School Official (DSO) at the Office of International Student will process your request. 

USCIS Forms

All USCIS forms can be downloaded directly from the www.uscis.gov/forms to ensure that you are using the latest version. Many applications can be filed online rather than by mail.  

  • FORM I-34 - for use by your financial sponsor, if your sponsor is in the U.S.  Your sponsor should sign this form if they agree to provide financial support to a beneficiary of certain immigration benefits (in this case, their support for you as and F-1 student) for the duration of their temporary stay in the United States.  

  • FORM I-539 - Use this form if you are requesting a change of status to F-1 from another visa status OR if your SEVIS record has been terminated and you are requesting reinstatement of status (ROS).

  • FORM I-765 - This form is used to request employment authorization (Pre/Post Completion OPT, Severe Economic Hardship)
