Accommodations Request Form
Please submit this form if you are a student who wishes to request accommodations
for the 2026 Commencement Cermony on Friday, May 22nd, at 5:00 pm. It is not necessary
for guests to request accommodations in advance, unless they need the program in an
alternate format. Please see the Commencement Guest Information webpage under Disability Accommodations for further information.
All requests should be submitted no later than May 8, 2026, to ensure adequate preparation
time.
