Alternative Media Request Form

Alternative Media is produced for Cerritos College Students, Faculty, and Staff with a verified disability. Items to be converted for a Student must belong to the Student and be needed for a Cerritos College course in which they are enrolled. Validation of a request may take two to three working days. Please keep your requests clear and concise. Validation of each request will be as soon as possible.  We welcome your comments and suggestions.

• Indicate the Title, Edition, 13 and/or 10 digit ISBN of the work to be converted

• The names of the book's author and  publisher

• The semester required

          If you have any other information pertinent to the captioning of this video such as a presentation date required by or that you are the creator and we have your permission to caption.
