  • This step is required for scholarship and self-pay students
  • The Intake Form begins your journey and allows us to guide you to graduation and award you the Industry Retail Management Certificate.
  • Note: you will need your Associate/employee ID # to complete the intake form.

  4. To complete the intake form, you must identify the partner college you will be attending.

Selecting your College

  1. For “College you will attend.”  Choose Cerritos College.
  2. When you have completed the form, click the “SUBMIT” button found on the green band at the bottom right of the application.
