Required Immunizations

  1. Documentation of Tetanus, Diphtheria and Pertussis (Tdap) booster within 10 years
  2. Proof of immunity by titer for the following:
    • Hepatitis B (presence of anti-HBc and antiHBs)
    • Rubeola
    • Mumps
    • Rubella
    • Varicella
    • Seasonal flu
  3. Covid-19 complete vaccine regime
  4. TB skin test or QuantiFERON gold blood test

If any titer shows a negative response, appropriate immunizations are required and FOLLOW-UP TITERS MUST BE COMPLETED.
