What is Disability Documentation?
In order to support your request for accommodations related to a disability, you must provide DSPS with some documentation of that disability. Following are some types of documentation we accept:
- Your most recent IEP and Psycho-Educational Report. You might have it at home or you can get it from your high school district student records office, if you received special services in high school;
- A report from your medical doctor or an agency that has your records, such as Department of Rehabilitation or Veterans Affairs;
- A report or from your psychologist/therapist/psychiatrist or other professional;
- A report from your eye doctor or other medical specialist;
- An audiogram;
- DSPS can provide documentation of your disability if you have a physical condition, such as a broken leg, which can be verified by visual observation of the condition.
If you don't have any of the above documentation, you can take the Disability Documentation Form to your doctor or other professional to fill out.
