Preferred Name Change Process

Many people use preferred first names rather than their legal first name.  Students and employees who have a preferred name as a result of their gender identity are encouraged to submit a preferred name change form.  Please note first name changes will only be approved as a gender identity accommodation.  Each request is reviewed on an individual basis.  Requests that are deemed by the Title IX Coordinator to be inappropriate, offensive, or for the purpose of misrepresentation, will be denied.  It is significant to note that some systems (ex. payroll) are unable to be changed as it must reflect a person’s legal name. Information regarding the name change process for students and employees is below.

For students:

Students who wish to be referred to by a preferred name as a result of their gender identity, should complete the Student Preferred Name Change Request Form

Upon receipt, the Title IX Coordinator will contact the student and provide them with information regarding their rights, resources, and accommodations.  The student will determine what accommodations they would like.    

For employees:

Employees who wish to be referred to by a preferred name as a result of their gender identity, should complete the Employee Preferred Name Change Request Form

Upon receipt, the Title IX Coordinator will contact the employee and provide them with information regarding their rights, resources, and accommodations.  The employee will determine what accommodations they would like.
