As of May 20, 2023, employees are no longer required to complete a daily screening when visiting campus.  Optimum HQ remains available for self-reporting positive cases of COVID-19.  Employees who test positive for COVID-19 should submit an online self-reporting form to Human Resources.

All employee-related communication for COVID-19 should be directed to

The following steps for an employee will take place after a positive test result for COVID-19 has been confirmed:

  1. Employee will self-report their positive test to Human Resources using the online reporting form on Optimum HQ.
  2. Employee will be contacted by Human Resources for contact tracing after self-reporting.
  3. Employee will receive follow-up information from Human Resources for returning to work.
  4. Employee will complete the isolation period as determined by Human Resources prior to returning to work.
  5. Employee is cleared by Human Resources to return to work after all required protocols are met.

How to report for a positive covid-19 test

The following steps outline how to submit an online self-reporting form for COVID-19:

  1. Log in to your Cerritos Campus Portal.
  2. Select the "COVID-19 Self-Reporting Tool" icon.
  3. You will be redirected to the Optimum HQ website.
  4. Log in to Optimum HQ.
  5. Complete the online self-reporting form questions.
